*splash* into the river
Sinking
dropping further and further
reaching out
you're alone
and nobody realizes
you're helpless
You're sinking
slowly and painfully
but nobody realizes
you cry out
and choke
water fills your lungs
*thud* you hit the bottom
someone sees you
but it's too late
you're too far gone
and can't be brought back

When I wrote this I was thinking about how you can do so many self-destructive things without anyone around you noticing
